How to Obtain a Passport for a Child As Evidence of Citizenship

1). Gather proof of the minor's citizenship. A birth certificate, consular report of birth abroad or certification of birth, naturalization certificate or certificate of citizenship will satisfy this requirement.
2). Gather documents to substantiate the relationship between the child and parents or guardians. Documents that will satisfy this requirement are the minor's U.S.-issued birth certificate, foreign birth certificate, consular report of birth abroad or certification of birth, adoption certificate, or court orders establishing custody or guardianship. It is important that both parents are named on the documents.
3). Print out and complete form DS-11, the U.S. passport application. Do not sign it. Parents are required to be present and to sign at the passport acceptance agency before an agent.
4). Photocopy the government-issued identification cards of each parent.
5). Call or visit the U.S. Department of State's website to schedule an appointment. When scheduling, you have the option of selecting a passport acceptance facility where passport photos will be taken at the time of application, or you can present your own 2-by-2 -inch photo of the minor.
6). Submit the application to the passport agent with the supporting documentation and appropriate fees. Processing time is normally 4-6 weeks. The U.S. passport will serve as evidence of the child's citizenship.
