How to Replace a Lost Passport in the USA

      If you lose your U.S. passport while in the United States, there are steps which must be taken to first report then renew the lost document. Once you have realized that your passport is gone, do not delay. Call the U.S. State Department Lost Passport Hotline at 1-877-487-2778. This action will void your current passport and will make it invalid for travel even if you should find it.

    • 2). Locate your local Passport Agency or Acceptance Facility using the U.S. State Department online locator service (see Resources).

    • 3). After locating the most convenient renewal facility for you, look up, print and complete the U.S. State Department form DS-64 (Statement Regarding a Lost or Stolen Passport), and form DS-11 (Passport Application), both available online (see Resources).

    • 4). Bring the following items to your appointment at the local Passport Agency or Acceptance Center:

      Your Completed form DS-64

      Your Completed form DS-11

      Current Proof of U.S. Citizenship

      Proof of Identity

      Two Recent Color Passport Photos

      Current Fees as dictated by the U.S. State Department in the form of a check, money order or major credit card made out to "United States Department of State."
