How to Get My Birth Certificate in Idaho
- 1). Write a letter to Idaho Bureau of Vital Records and Health Statistics requesting a duplicate birth certificate. Include the type(s) of certificate being requested, the full name on the certificate, date of birth, city of birth, parental names, purpose for certificate, number of certificates ordered and return address. The letter must also include the signature of the requester. Alternatively, you can complete the Idaho Vital Statistics Certificate Request form on the website listed in Reference.
- 2). Make a photocopy of the required identification and attach it to your request form or request letter. If you are applying for a minor or relative, proof of your relationship may be required. Acceptable identification includes any picture ID with a signature or two forms of secondary ID, such as a social security card or school ID. At least one must contain a signature.
- 3). Attach a check or money order for the current fee. Other fees may arise during processing and the Vital Records and Health Statistics office will contact you in that event.
- 4). Mail or fax all required paperwork, including the letter or form requesting the birth certificate, a photocopy of your identification and a check or money order to:
