The role of non-dietary therapies—as an adjunct or as an alternative to the GFD—has yet to be ascertained. Of the candidate approaches, immunotherapy (including hookworm exposure) is currently explored as an alternative to GFD but, even if successful, is unlikely to benefit all patients. The role of glutenases (propyl endopeptidases) and tight junction regulators (Table 5) are likely to be in reducing the threshold response and optimising the benefits of gluten restriction rather than allowing a normal gluten-containing diet.
Table 5 contains a list of potential novel treatments. None of the available novel treatments can as yet (January 2014) be recommended for use outside clinical trials.
