Vitamin K Antagonist in Hypertrophic Cardiomyopathy and AF
HCM occurs has a prevalence of one in 500 and has many genetic mutations. Among rhythm disorders, AF is the most common sustained arrhythmia coexisting with HCM. Given the fact that the risk of stroke in HCM patients with AF is high, antithrombotic therapy with warfarin is recommended in national guidelines, regardless of the presence or absence of other risk factors for stroke, for example, CHADS2 score. There are no data supporting the use of dabigatran, rivaroxaban or apixaban in this population. Our goal in this manuscript is to review and emphasize the high prevalence of AF in patients with HCM, and to underscore and increase awareness regarding the importance of anticoagulation in this setting.
