- 1). Find your copy of the SSI application. You may need to give the representative more information than your birth date, Social Security number and name.
- 2). Call either your local Social Security office or (800) 772-1213. Enter your Social Security number when prompted and either listen to the Health Insurance Portability and Accountability Act (HIPAA) or agree you were offered a chance to listen to your HIPAA rights. HIPAA rules protect patient's rights to confidentiality. You will then be passed on to a representative.
- 3). Answer the representative's questions to verify you have the right to access information about the application. Ask the representative about the status of your SSI application.
